Fall Athletic informa tion 2014- 15 School Year

I’d like to take the opportunity to welcome new and returning students to the Academies of Frederick Douglass High School. Although it’s the third week in July, the start of the fall season is right around the corner. If you plan to participate in our Fall Athletics Program please take care of business over the summer. In this packet you will see information for all of our Fall Sports. Please review it carefully.

Tryouts for Fall Sports start on August 13^th. In order to be able to participate, athletes must turn in all completed paperwork to the Athletic Director by Friday August 11^th. Prospective Athletes that do not turn in their completed parent Permission and Participation Physical Evaluation by August 9^th will not be permitted to try out for a fall sports team. There will be no Exceptions. Paperwork may be dropped off throughout the summer up until August 11^th to the main office during summer hours. There will be a box placed in the main office marked fall Athletic Sports Forms. When completing the parent permission form, please make sure that the Health Insurance information, including the company and policy number are included. Additional forms may be required for individual teams which may be obtained from the coach during the Meet the coach Meetings held in May or June( see dates below) or at the first day of try-outs.

Football Meetings: A separate letter will be sent to all students with dates.

All sports – August 4^th, 2014 6pm

Massive Baltimore City High School Sport Physical Day at Union Memorial Hospital: August 9^th @8a.m Cost $10.00. For One day only.

Please do not call union memorial Hospital to Schedule an appointment.

August 14th – fall tryouts Begins- see attached schedule
